(NOTE: If you paid with a credit card, your deposit will be refunded to the same card). If money is to be wired-transferred, please include in this box, Bank Name, Bank Address, Account Number, and ABA or SWIFT code (routing number). By providing this information, you hereby authorize us to deduct from your deposit a $45 fee for the bank transaction.
