Before using this form, you MUST verify that DegreeWorks is displaying the correct Program of Study (Major, Minor, etc), and Bulletin Year. If it is not, please submit a Change of Major request through C-SAS before completing this form.

Degree Auditors cannot review your worksheet until these changes have been made and your worksheet accurately reflects your Program of Study and Bulletin Year.

You must indicate DegreeWorks is displaying the correct information by selecting "Yes" for the Program Verified option. Programs that have not been verified will NOT be reviewed. Please identify specific problems you need addressed on your worksheet in the Comments section.

  • Pre-Professional Programs are reviewed by the Pre-Professional Office located in Sierra Vista Hall, room 111. Please call 951-785-2452 for more information.
  • Please be aware that review requests made during the spring and summer quarters will take longer to process due to graduation.
  • Once your audit has been reviewed you will receive notification via email.
  • If you have questions regarding this process, please call (951) 785-2006 to speak to your Degree Auditor.
