Instructions for IRB application

If your research involves human participants, you need to get an IRB approval before your study begins. However, if your research is a class project, and the data collection stays in the classroom and the research results would not be presented or published outside of the classroom, you don’t have to get an IRB approval.

Please find the following IRB forms below: IRB Initial Application Form, Amendment Request Form, and Renewal Request Form. Please note this form is a dynamic, interactive PDF document. When using it you should save a copy for your records before submitting it to the IRB. Send the complete forms to the IRB office (Ambs Hall 150), or email the forms to irb@lasierra.edu.

If you are a student and send your application electronically, send your complete application to your faculty supervisor  (in addition to your sending this to the IRB), and ask him/her to send his/her approval to irb@lasierra.edu (This is in lieu of the faculty supervisor’s signature.)

Please include (attach) all of the informed consent form(s). If your research does not use a written consent, please indicate how you will obtain participant consent.

In case of research involving children include both forms used to solicit the assent of the children and the permission of their parent(s) and/or guardian(s).

Please include (attach) all the questionnaires, surveys, or any other data collection protocols with this application. Please include (attach) the certificate of completion for the free Panel on Research Ethics Training that can be found at TCPS 2: CORE.. The certificate of completion for this training is good for five years. This requirement is for all the IRB applicants except:

If you are a student of a class that involves research with human participants and needs an IRB approval for the class project (i.e. PSYC 323, EXSC 364, etc.), ask your instructor about this training. It is up to the instructor of this class for the student training requirement.

If you are an instructor of a class of which students need an IRB approval or if you are a faculty advisor of a student(s) who will need an IRB approval, you, as a faculty advisor or instructor, need to complete this training and send the certificate to the IRB.
