  1. List each swimmer’s age (as of 6/23/14), gender and name
  2. Choose Session Dates (1-4) and Lesson Type:
    G = Group Lesson
    P = Private Lesson
  3. Choose a lesson time. Classes are ½ hour in length. Choose a half-hour lesson during the following time blocks: 9:00-11:30 am, and 3:30 -6:30 pm.
  4. Choose a class level using the class level guide below
  5. Fill in ALL Responsible Adult Information
  6. Complete Payment and Release information
  7. Sign and Date the form

Parent & Child: All about getting the child comfortable and familiar with the water. Floating and gliding on the front/back, introducing arm and leg action, successful submerging, and life jacket safety. We ask that a parent be in the water with the child at all times.

Level 1: Entering/exiting pool, submerging/blowing bubbles, float with minimal assistance on front and back, introduce treading, swimming on front and back with arm and leg action with assistance, water safety rules. Exit skills: entering unassisted, move 5 yards, bob 5 times, and float on front/back with assistance.

Level 2: Submerge and open eyes to pick up objects, bobbing, floating without assistance, and gliding, treading motions, swimming movements on front and back, and water safety. Exit skills involve front/back float for 5 seconds each without assistance, pushing off and gliding/swimming for 15 yards on front/back unassisted.

Level 3: Introduction to diving, rotary breathing, gliding, floating, and treading unassisted, front/back crawl, water safety rules and survival, and helping others. Exit skills include swimming front crawl 25 yards with face in water and rotary breathing, treading or floating for 30 seconds and back crawl 15 yards.

Level 4: Diving from basic positions, swim underwater, surface diving, floating, pushing off and gliding, treading, front crawl, breaststroke, back crawl, elementary backstroke, introduction to butterfly, swim on side, jump from height with life jacket, water safety, and helping others. Exit skills include front crawl/back crawl for 25 yards, breaststroke/elementary backstroke for 15 yards, floating/treading for 1 minute.

Level 5: Diving, gliding, surface dives, floating, introduction to flip turns, treading water, all 6 strokes, water safety, and helping others. Exit skills include front crawl/back crawl for 50 yards, elementary backstroke/ breaststroke for 25 yards, float/tread for 2 minutes.
