Frequently Asked Questions

• Every new student, if he or she is taking 8 units or more, should have a Physical Examination record turn in to the Student Wellness Services. The Physical should be within one year (we accept transfer student physicals if done within one year). The Physical should include the Immunization Record with the following vaccines:

  • Hepatitis B -- Three doses series required
  • Measles, Mumps, Rubella -- Two doses required
  • Tdap -- Within ten years required
  • Meningococcal vaccination -- Recommended, especially for Freshmen living in the Residence Halls who wish to reduce their risk of meningococcal disease.
  • Tuberculosis Screening -- PPD (Mantoux) required, within the past 12 months. (Tine or Monovac not acceptable). If PPD is positive, a chest X-ray required.


The student can have these done at the Student Wellness Services for $100.00; tNew students are granted a quarter (about 12 weeks) to provide us with this pre-matriculation requirements for La Sierra University Student Wellness Services,. If we do not receive all the required documents, then the student will have a HOLD for the next quarter. To access the required forms for entrance, visit our Vaccination Policy page.

Counseling Services' No-Show Policy

Due to our waitlist and high demand for services, our no-show/late-cancellation policy is as follows: If you would like to cancel or reschedule your counseling appointment for any reason, you need to let Student Wellness Services know at least 24 hours before your appointment.   If we do not hear from you, we interpret that to mean you are choosing to disengage from counseling at this time. As such, you will be removed from the therapist's caseload. If/when you are ready to re-engage in therapy, please give us a call and we will once again place you on our waitlist.
