For Faculty

The Annual Faculty Report is due June 24 each year.  Your Annual Faculty Report will be used by your Academic Department Head for the Department Annual Report. You may choose from three versions:

• Annual Faculty Report - Narrative Version Outline
• Annual Faculty Report - PDF Fill-in Form Version.  Note:  To save your information, be sure you are viewing the file in Adobe Reader 9 or later, or other viewer software that allows you to save your form data.
• Annual Faculty Report - Microsoft Word Fill-in Form Version.  Use this is your computer won't let you save the form data for the PDF version above.

Each year, faculty review the teaching of their peers as arranged by their department head.  This is the recommended template.
