Pre-Entrance Requirements and Vaccination Policy

All students must complete and submit the following items to our office before enrollment. The forms and supporting documents can be submitted in person, by fax, or by email. Please keep in mind that communications via email over the internet are not secure. Although it is unlikely, there is a possibility that information you include in an email can be intercepted and read by other parties besides the person to whom it is addressed. Please note: students who do not provide these items within the first quarter attended will have a hold placed on their account.

Pre-Entrance Requirements & Forms

Student Athletes (planning to try-out for/join any university sports team):

Vaccination Policy

  • Tuberculosis Screening Questionniare – PPD (Mantoux)* -- Required, within the past 12 months. If the screening determines that you must get a PPD, and it is positive, a chest X-ray/QuantiFERON-TB Gold blood test is required.
  • Hepatitis B (Hep B) vaccine series* – Three doses required, if under 19 years old
  • Measles, Mumps, Rubella  (MMR)* vaccine series – Two doses required
  • Tetanus, diphtheria, pertussis (Tdap)* vaccine – One dose required, within the past ten years
  • Meningococcal Conjugate (Men-ACWY)* vaccine – Required for all students living in the Residence halls

    *The student can have these done at the Student Wellness Services. Please contact the department for more information.

    VACCINES EXEMPTIONS PROCESS - To apply for an exemption, please download and complete the appropriate exemption form below. The deadline to turn in the exemption form (and any supporting documents) is 2 WEEKS BEFORE YOU MOVE INTO A RESIDENCE HALL OR BEGIN CLASSES OR BEGIN EMPLOYMENT.
  • Medical Exemptions Form for all vaccines (Hep B, MMR, Men-ACWY, Tdap)
