Tuition Waiver Authorization and Transcript Release

This form is to be completed by Pacific Union Conference teachers planning to take courses at La Sierra University. The completed form is to be submitted to your superintendent of schools, who will forward it to La Sierra University's Records Office. Once this form has been received and processed, course registration instructions will be sent to you by email from the La Sierra University Records Office. To ensure timely processing, this form must be submitted 5 business days prior to the first day of the desired quarter.

An authorization form must be submitted for each term (quarter) of attendance.

The above named teacher who is currently teaching and meets the provisions of the Pacific Union Conference Education Code Section E10-152, is authorized to attend:
Intended quarter you want to start at La Sierra University.
Intended year you want to start at La Sierra University.
Pacific Union Conference teachers who are authorized to attend on-campus or online* classes in the School of Education are given a scholarship to cover general fees and up to 12 quarter hours of tuition per school year. The academic school year begins with the Summer term and ends in the Spring term. The scholarship covers all 500 level courses which meet credential renewal requirements, or degree completion requirements for the M.A. degree and/or professional certification as part of a M.A in teaching, administration, counseling psychology, special education, and child development. Additional information can be found in the PUC Education Code Section E10-152. The following terms apply to all teachers receiving this La Sierra scholarship:
  1. Tuition charges or penalties resulting from the withdrawal of a class are the responsibility of the teacher.
  2. Unless approved by the Union and the University ahead of time, courses taken outside of an approved program are not eligible for the scholarship and must be paid in full by the teacher.
  3. Non-passing grades may result in the loss of the scholarship. A grade of a B or higher is considered passing for most classes at the graduate level.
*Teachers registering for an online course must also fill out and submit the “Online Advisor Approval”.
