Covid-19 Testing on Monday and Tuesday!

Good evening campus! I hope you all are doing well and finding ways to successfully manage the very present election anxiety.

We have one more covid testing event on the calendar for this quarter.  Here is the info and a link to pre-register.

When?  Monday, November 9 & Tuesday, November 10

Where? Gym (Alumni Pavilion)

Please remember to print, fill out and bring with you the following forms:
 

  1. Covid-19 Patient Test Request Form
  2. Authorization for Release of Medical Information
  3. For those wanting to do the antibody test - Venous Consent Form
  4. For those with health insurance: A physical copy of your health insurance card
  5. For those without health insurance: Uninsured Patient Form, a copy of your social security card and gov. issued ID.
