Forms for Covid Testing Event

Hi all, It's great to see so many of you signing up!  Here are a few more pieces of import pieces of information as you prepare to come:

 

  1. Required forms to print, fill out and bring with you to the event are attached to this email.
  2. ALL La Sierra University faculty, staff, students are welcome to attend the event.  You do not need to be classified as an "essential worker" to enter campus to be tested.
  3. Family members are welcome to sign up as well.  Just be sure to have each person fill out the forms and bring copies of their health insurance and ID.
  4. When you enter campus, inform the wellness check assistant that you have an appointment to be tested and they will give you a special entrance sticker.
