Testing Events Next Week: Sign up by tonight!

Hi campus family, Dan Nyirady, Director of Student Wellness Services, and I would like to remind you of the upcoming testing event that will be held on campus.  

Pre-registration is required so you will need to sign up online in order to participate.

When?  2 days - Monday, October 12 or Tuesday, October 13

Where?  La Sierra University - Founders Green

How much?  FREE - bring a PRINTED COPY of your health insurance card and ID

There are 2 types of tests available.  

1. PCR (nasal swab) - required for everyone who needs campus access

2. Antibody (blood draw) - optional 

Remember, bi-weekly testing is required for the following groups of people to maintain campus access:

  • Students and living in residence halls
  • Student athletes attending practice on campus
  • Students attending in-person classes/labs on campus
  • Employees working on campus more than 4 hours per week

Everyone will need to print out and bring with you the following:

  • Covid-19 Patient Test Request Form
  • Authorization for Release of Medical Information Form
  • A copy of your health insurance card (front and back) and ID

For those that sign up for the Antibody test (blood draw) please also bring:

  • Venous Consent Form

For those without health insurance please also bring:

  • Uninsured Patient Form
  • A copy of a government issued ID (drivers license, SS card, passport, etc)

If you cannot attend this event but need campus access, you will need to have a PCR test at another facility and send the results to covidresults@lasierra.edu before Monday, October 19.
