Provost Appeal Form

The purpose of the Provost appeal is: 

• to request an exception to a decision made by an academic support office
• to appeal a decision initially made within the Office of the Dean of the College or a School

The Provost Appeal should not be used for financial appeals, Student Life appeals, academic appeals, or student employment appeals, except in cases where all steps in the above-described processes have been completed.

You can fill out the form electronically and send it to the Provost's Office ([email protected]) or print the form and submit it in person to Admin Building 225.
